There are 2 ways to register: online or paper. Online registration is preferred and strongly encouraged.\n\n\n\nVaccines that will be offered this year:  \n\n\n\nFlu\nHigh Dose Flu (for ages 65 and older)\nCOVID-19 (12 years and older)\nRSV (75 years and older)\nShingles (does 1 and dose 2)\nPneumonia\nT-dap\nMeningitis\nHepatitis B\nMMR\nPolio\nVaricella\n\n\n\nIf you request any other vaccinations\, Pathways Wellness Program will contact you directly if there is an out-of-pocket expense due to copays or deductibles. \nOnline Registration: here is the registration link where you can sign up to receive vaccines. Once you click on the link\, you will have the option to choose from a variety of vaccines that will be offered. The last day for online registration will be 9/22/2025. The link will close after that date so be sure to sign up before then.  \nPathways Wellness will collect all insurance information prior to the clinic. If you do not have insurance you may choose to pay the cash rate for the immunization – here is the Cash Rate Fee Schedule.
