  • Time: 1:00pm – 5:00pm
  • Overview: Provident Charter School Central will host a Vaccine Clinic for staff, students, and their families on Wednesday, October 1st from 1:00pm-5:00pm. There are 2 ways to register: online or paper. Online registration is preferred and strongly encouraged.

Vaccines that will be offered this year: 

• • Flu
  • High Dose Flu (for ages 65 and older)
  • COVID-19 (12 years and older)
  • RSV (75 years and older)
  • Shingles (does 1 and dose 2)
  • Pneumonia
  • T-dap
  • Meningitis
  • Hepatitis B
  • MMR
  • Polio
  • Varicella

If you request any other vaccinations, Pathways Wellness Program will contact you directly if there is an out-of-pocket expense due to copays or deductibles.

Online Registration: here is the registration link where you can sign up to receive vaccines. Once you click on the link, you will have the option to choose from a variety of vaccines that will be offered. The last day for online registration will be 9/22/2025. The link will close after that date so be sure to sign up before then. 

Pathways Wellness will collect all insurance information prior to the clinic. If you do not have insurance you may choose to pay the cash rate for the immunization – here is the Cash Rate Fee Schedule. The Pathways Wellness staff will bring preprinted consent forms for you to sign on site.  Most insurances cover the standard flu shot.

Paper Registration: paper registration is welcome by completing the following form(s)

• • Patient Walk In Consent Form (for anyone 18 years of age or older receiving vaccines)
  • Walk In Minor Consent Form (for anyone under the age of 18 receiving vaccines; must be signed by parent or legal guardian)
